We have expanded to provide similar services at Palm Bay Community Hospital. BES Media Notification
Brevard Emergency Services, P.A. is today providing notice to patients and the media that it had an unauthorized disclosure of patient information on August 26, 2011 for 2,200 patients seen between July 16, 2010 and June 15, 2011 in the emergency department of Holmes Regional Medical Center. Brevard Emergency Services is the medical group contracted with Holmes Regional Medical Center to provide emergency services at the hospitals' emergency department.
Brevard Emergency Services takes seriously its obligation to keep patient information confidential. Upon learning that documents were missing which contained patient information, Brevard Emergency Services investigated the matter. Information used to survey patients regarding the quality of medical services received from its physicians was taken from the corporate office without permission by a terminated employee. When contacted, the former employee readily returned the documents to Brevard Emergency Services. We do not believe that this information was subsequently shared with others, but to be cautious, we have provided notice to patients and the public.
The information contained on the documents includes the following: Patient Name, Patient Telephone Number, Date of Services, Patient Account Number, Provider Name, and Location of Service. The following information was NOT contained on the form: Social Security Number, Date of Birth, Patient Address, Patient insurance information, and Treatment or Diagnosis Information.
As a result of the disclosure, Brevard Emergency Services has engaged the services of Experian to provide Identity Theft protection for patients who desire to take advantage of Experian's ProtectMyIDTM product at no cost to the patient for one year. This product helps detect possible misuse of personal information and provides the patient with identity protection services focused on immediate identification and resolution of identity theft.
